Application for the Use of Human Tissue/Blood/Body Fluid for Research Purposes

This new form was developed to ease the application burden on investigators wanting access to specimens for research purposes. Please note that if you need to characterize the specimens with personal health information, you will also have to complete the section of the form entitled 'Request to Access Retrospective Data for Research Purposes'. This form replaces the longer TAHSN form for these specific purposes.
