Quality Improvement Plan

In June 2010, the Ontario Government passed the Excellent Care for All Act. This legislation will help support hospitals to further improve the quality and safety of care we provide. One of the ways that the Excellent Care for All Act is helping hospitals meet our communities’ expectations regarding quality, patient safety and accountability is through the public reporting of Quality Improvement Plans (QIP).

Quality improvement is a constant priority that helps us continually find new and better ways of doing things so that we can enhance care for patients, increase satisfaction and achieve even better clinical outcomes. The QIP is one of the tools that we are using to help us document and review our current performance in a variety of areas such as quality, efficiency, patient safety and financial health.

This plan will help us to focus on selected areas for improvement and to chart our progress. The QIP provides a meaningful way for Mount Sinai Hospital to clearly articulate our accountability to our community, patients and staff. The Mount Sinai Hospital QIP is focused on creating a positive patient experience and delivering high quality health care.

Our QIP is made up of two parts:

  1. A document provided by the Ministry of Health and Long-Term Care that includes a brief overview of our Quality Improvement Plan for 2012/2012, highlighting and listing our hospital’s top priorities for the year. 
  2. A  document provided by the Ministry of Health and Long-Term Care that includes our priority improvement targets and initiatives for 2012/2013. We have indicated the specific actions and targets we have set for these priorities, which encompass quality and safety, access and efficiency, the patient experience and financial health.

The QIP reflects Mount Sinai’s values of providing patient-centered care using a collaborative approach with a focus on continuous improvement.  Every element of the plan has been informed by our key strategic priority of Putting Patients First which means that safety, quality and the patient experience underscore all aspects of our service delivery.
