Mount Sinai Hospital is committed to openness and transparency with its patients and community. You can make requests for health records or corporate information.

Health Records

The Personal Health Information Protection Act, 2004 (PHIPA) is the law that grants patients the right to access their own health records. This law also requires Mount Sinai Hospital to protect patient health records and keep them confidential. Corporate Records

The Freedom of Information & Protection of Privacy Act (FIPPA) provides a broad right of access to certain Hospital records. It also requires the Hospital to keep personal information confidential.
