Quality Improvement Plan-OLD

In June 2010, the Ontario Government passed the Excellent Care for All Act to support hospitals to further improve the quality and safety of care we provide. One of the ways that the Excellent Care for All Act is helping hospitals meet our communities’ expectations regarding quality, patient safety and accountability is through the public reporting of Quality Improvement Plans (QIP).

Our Quality Improvement Plan helps us drive performance and enhance patient care through focused targets and actions related to five specific dimensions of quality:  care that is accessible, integrated, safe, effective and patient-centred.

Aligned with system and provincial priorities, the QIP has Board oversight and is reviewed regularly, throughout the year.  

As a result of the newly formed Sinai Health System, which has brought together Mount Sinai Hospital, Bridgepoint Active Healthcare and Circle of Care to deliver a more seamless care experience for our patients, we have developed a QIP overview that reflects the new organization’s priorities. We are building the Sinai Health System into a premier exemplar of an integrated health system for the future that follows the patient journey from acute care, complex and rehabilitative care, primary care, home care, and other community-based services to create an integrated continuum of care. As the system evolves, our QIP will help us to focus on selected areas for improvement and to chart our progress.

Our QIP is made up of two parts:

  1. A document provided by the Ministry of Health and Long-Term Care that includes a brief overview of our Quality Improvement Plan for 2016/2017, highlighting and listing Sinai Health System’s top priorities for the year. 
  2. A  document provided by the Ministry of Health and Long-Term Care that includes Mount Sinai Hospital’s improvement targets and initiatives for 2016/2017. We have indicated the specific actions and targets we have set for these priorities, which encompass quality and safety, access and efficiency, the patient experience and financial health.
