When you're coming for an appointment in Audiology, please remember to bring:

• your referral form, signed by your physician
• your Mount Sinai Hospital card

If you do not have a Hospital card, you can get one in the Admitting Department on the main floor of the Hospital (see map). If you are doing this the same day as your appointment, please allow at least 15 extra minutes.

Directions

Mount Sinai Hospital is on the west side of University Avenue, between College and Dundas. The closest subway is Queen’s Park, on the University line at College Street. Walk south to Mount Sinai Hospital. More information »

Audiology appointments take place on both Floor 2 and Floor 4 of Mount Sinai. Please check your appointment card for your room number.

For Room 201, take the elevator to level 2 – one floor BELOW main floor, and follow the signs to Hearing and Balance Unit.
 
For Room 405, take the escalator (in the main lobby) or an elevator to Level 4, and follow the signs to Audiology.
