Referrals:

Patients must be referred by their dentist (DDS), or physician (MD).

Fees:

Consultation and examination fees (including dental x-rays) and any treatment are NOT COVERED BY OHIP.

Patients must forward/bring all pertinent radiographs and appliances to their consult appointment.

Once we receive a complete/detailed referral, please allow 6-8 weeks for your patient to receive a detailed letter with instructions, along with the date and time of the scheduled appointment.
