Appointment Checklist

In preparation for your appointment, please review the following:

Complete the following steps BEFORE your first appointment

1. Download and complete our medical history questionnaire and bring to your appointment

2. On the Day of Your Appointment and in preparation for your first visit (Download this checklist)

  • Please arrive 45 minutes before your appointment. For directions to Mount Sinai Hospital view the Google map.
  • All patients seen in the hospital must obtain a hospital card prior to the first appointment. You must have your Ontario Health Insurance Plan (OHIP) Card.
  • If this is your first visit to the hospital or you have lost your card, please go to “Patient Registration” located on the main floor of the hospital next to the Murray Street entrance.
  • After getting your hospital card, please go directly to the Dental Department, located on the 4th Floor, Suite 412 (take the main floor escalator to the 4th floor, Mezzanine level and turn right).

3. Please BRING the following items with you to your first appointment:

  • Radiographs / Reports NEEDED FOR YOUR APPOINTMENT
  • Completed Medical History Questionnaire
  • LIST OF MEDICATIONS YOU ARE CURRENTLY TAKING
  • Valid Ontario Ontario Health Insurance (OHIP) Card
  • Mount Sinai Hospital Card (see above information to obtain a card)
  • Method of payment: Visa, Mastercard, Interac or cash for payment of professional fees following your appointment. Learn more about fees and billing.

 

APPOINTMENT INFORMATION

We understand the value of your time and therefore we do our very best to stay on schedule. Our clinic provides multiple types of dental services and occasionally due to the nature of our clinic, emergency situations can occur and cause delays. If this occurs, your patience and understanding is greatly appreciated.

"NO SHOWS" AND CANCELLATIONS

A scheduled appointment is time that is reserved just for you. Please be aware that we require 72-business hour notice to cancel or reschedule an appointment. This will allow us to schedule another patient in need of treatment. Failure to provide sufficient notice may result in a cancellation fee.
