Patient Checklist for your first consultation appointment

To help simplify your first consultation appointment, we have the following suggestions:

• Where applicable, please come as a couple.  
• Please bring your completed Infertility History Sheets below and any copies of test results you may have, to your appointment.

1. Male Fertility History

• Please ensure you have a valid Health Card (both partners where applicable). To renew an Ontario Health Card, visit http://www.health.gov.on.ca/english/public/pub/ohip/card.html. 
• Certain tests and fertility enhancement programs require payment. For your convenience, all payments may be made with cash, credit or debit card.

• *HIV testing is a standard requirement for acceptance into our programs. 
• Lab results may take up to 4 weeks (download contact information for labs - 89 KB pdf )
• All payments can be made with cash, credit or debit card.

Missed Appointment Policy

We ask you to show consideration by notifying our in advance if you are unable to keep an appointment. We would like to have the option of offering that appointment to another patient.

If you do not notify our office at least 24 hours in advance to cancel or rebook an appointment, you will be charged the following amount:

• Missed Consultation Appointment Fee: $75 (No Show or less than 48 hours cancellation notice) 
• Missed Review Appointment Fee: $35 (No Show or less than 24 hours cancellation notice) 

Please note these fees are not covered in our Administration Fee. These fees have been approved by the College of Physicians and Surgeons of Ontario, and are OHIP-excluded costs.

If you are coming by TTC, we are a short walk west of the St. Patrick Subway Station.

If you are driving, there are public lots in the area as well as an underground lot below our building off of Simcoe St.
