Print off this algorithm and use it as a tool in your fertility assessments with your patients. It provides guidance on when to refer to a urologist, obstetrician/gynecologist or a fertility specialist.

Centre for Fertility and Reproductive Health Referral Form

An easy to use referral form, just print off and fax it to 416-586-4686. Please include all results as requested on the referral form. Your patient will be contacted by our Centre within two weeks of receipt of the referral.
