Then take the Murray Street elevators at the back, or west side, of the Hospital to the 12th floor.
 

Appointments

For information on how to make an appointment with a doctor, call our receptionist, Gloria Furtado at 416-586-8799.

Important: Before an appointment can be booked with one of the doctors, their office must receive a letter with appropriate relevant medical information (for example, test reports) from the doctor who is referring you.

The referring doctor may ask you to bring other test results, x-ray films etc. to your appointment. A hospital card and valid health card must be presented at all appointments.
 

Mammograms

To make an appointment for a mammogram, call 416-586-4422.

Important: You will be required to bring a requisition from your doctor when you come for your mammogram. A hospital card and valid health card must also be presented.
