Scotiabank Breast Cancer Transition to Health Program

This program is delivered by a nurse who facilitates the referral process from surgical to medical oncologist, to make it more efficient for our patients.

The nurse also creates individualized transition care plans for patients when they have completed cancer treatment for breast cancer. staying the course
 

What is a Transition Care Plan?

A transition care plan is an individualized written document about the patient’s diagnosis which can include details on:

  • treatment
  • follow-up care
  • symptoms to watch for
  • schedules for follow-up tests and appointments
  • possible late and long term side effects of treatment
  • available resources for emotional and psychological support
  • recommendations for general health and wellness, such as diet and exercise.
     

Who is eligible to have a transition care plan?

Patients who have completed their breast cancer treatment at Mount Sinai Hospital.
