Family Medicine
Our Family Health Teams combine the expertise and services of different health-care providers to help you address many kinds of health-care needs. We provide families with a range of comprehensive primary care services at any age.
We are committed to pursuing excellence and innovation in everything we do. We take a collaborative and multidisciplinary team approach to patient care and bring the best of what family medicine has to offer to our patients.
We have two Family Health Team locations:
Mount Sinai Academic Family Health Team - Toronto site
Our team is located in the Granovsky Gluskin Family Medicine Centre and integrated with Mount Sinai Hospital's Department of Family Medicine
Mount Sinai Academic Family Health Team - Vaughan site
Our team is located at the Sherman Health and Wellness Centre and affiliated with Mount Sinai Hospital's Department of Family Medicine
Our team is affiliated with the University of Toronto, Department of Family and Community Medicine. Research, teaching and quality improvement are an integral part of our work with staff and patients. Family medicine residents, medical students and other interprofessional health care learners are part of our team.
