Community Based Programs

We have programs that are based in the communities which we serve. See the referral criteria.

The Mount Sinai Hospital Ethno-Cultural ACTT provides intensive treatment for adults with serious mental illness with a goal to support recovery and the ability to live in the community by improving their quality of life.  The ACTT Program specializes in the provision of culturally competent and sensitive care to primarily Asian and South East Asian communities.

The interprofessional team of clinicians, including nursing, social work, occupational therapy, psychiatry and peer support, provides: :

  • Counselling
  • Case Management
  • Support with medication management
  • Assistance with activities of daily living
  • Advocacy and help with accessing community services
  • Psycho-educational and support groups

The Mount Sinai Hospital Mental Health Court Support (MHCS) Program responds to adults with an ethno-racial background who experience mental health difficulties and who are involved with the criminal justice system often as a result of their mental illness. 

Through a cooperative working relationship with other mental health court support workers at Old City Hall Court, Mount Sinai Hospital MHCS offers culturally competent and sensitive services to individuals by providing access to necessary treatment and support in the community. Through the concerted efforts in the community it is hoped that the future recidivism will be reduced or eliminated.

  • Those who have been charged with a criminal offense in the adult court (a Class One or Two Offense) and have issues related to their mental health.
  • Individuals who are part of an ethno-racial population

We offer the following services:

  • Assessment
  • Consultation and Advice for Old City Hall Mental Health Court
  • Brief Service and Case Management Support
  • Court Diversion, as appropriate
  • Education and Advocacy

The Chinese Seniors Wellness Centre is designed to meet the mental health needs of the Chinese community by providing assessment and treatment in a culturally appropriate, sensitive environment.

With a focus on early identification of mental illness, this Centre offers an array of culturally and linguistically appropriate programs, including psycho-education, health promotion, traditional and western health care that serve as an interface to more individualized mental health services.

The Clinic provides treatment for Chinese people who are over 65 years old and for families and caregivers who face challenges in caring for an older person.

Services are offered in Cantonese, Mandarin, and English.
