2015 Geriatrics Update Registration Options

1. Register by phone

   Please call the Geriatrics office at 416-586-4800 ext. 7674 with your credit card information.
   Note: if your call is not personally answered, please leave your name and number and  you will receive a return call within one business day.
    

2. Register by mail

   Download and complete the registration form (pdf )   Please make cheque payable to Mount Sinai Geriatrics. Bring/ mail your registration form and payment  to:

   Hours of Operation: Monday to Friday, 8:30 a.m. to 4:30 p.m.
      

3. Fax your registration

   Download and complete the registration form (pdf ) and fax your information to the Geriatrics office fax number - 416-586-5113.
    

4. Register online with PayPal
   Registration is now closed. 

CONFIRMATION OF REGISTRATION: 

You will receive a ticket via email that confirms your registration. You must bring this ticket with you on the day of the event as proof of registration. You are not considered registered unless you have received a ticket with your registration information included.
