Purpose

Care of the hospitalized person with diabetes poses many challenges, regardless of the primary reason for admission. This workshop will help clinical staff identify potential issues common in hospital areas and take action to prevent and/or manage acute complications of diabetes.
  

Objectives

Participation in this workshop will enable the participant to:

  1. Anticipate the effect of the illness or hospitalization on glycemic control. Take action to:
       a) Prevent severe highs or lows when fasting or unable to eat as 
            usual
       b) Assess and manage hypoglycemic or hyperglycemic states
       c) Identify glucose levels that require action
       d) Administer medications/insulin & support safe self-administration
  2. Assist a client who is initiating insulin to be able to:
       a) Use insulin pen and device
       b) Prevent, recognize & treat a hypoglycemic episode
  3. Identify client needs for discharge and coordinate discharge plans in collaboration with the team. Effectively use available inpatient resources including:
       a) Intranet clinical tools
       b) Referral procedures for inpatient nutrition and/or nursing consultation and/or local Diabetes Education Centre
           (DEC).
       c) Self-management education materials
       d) CCAC for Home Care
