Assertive Community Treatment Team (ACT Team)

Chinese Patient Programs

For adults experiencing persistent, complex mental health problems, our ACT Team provides culturally sensitive mental health outreach services.

We give care to people who have special needs, such as high demand for services, substance abuse, homelessness or involvement with the legal system.

Our team works with the clients to develop highly individualized treatment plans, provides practical support, and instills hope and trust.

This project is a joint venture with the Hong Fook Mental Health Association and a collaboration with the Centre for Addiction and Mental Health.

Referral from a hospital, physician or community agency is required.
