MyChart

MyChart is an online website where patients can create and manage their own personal health information based on clinical information that is available from participating hospitals and personal information that is entered by the users.

Some of the other hospitals using the MyChart system in the GTA include Sunnybrook, St, Michael’s, Baycrest and Michael Garron (formerly Toronto East General).

For example, if you are a MyChart user who sees a Cardiologist at St. Michael’s and has their regular mammography appointment at Mount Sinai Hospital, records related to both of these vists would be available to you.

Other hospitals in Ontario may have their own patient health information tool.

Mount Sinai Hospital patients can register for MyChart in a few different areas in the hospital. You’ll need to provide your email address to get a PIN for self-registration. You will also need to have your OHIP card with you.

1. At your next outpatient appointment in the following areas – just ask at reception
   • Diagnostic Imaging
   • Outpatient Laboratory
   • Outpatient Cancer Care Clinics
   • Marvelle Koffler Breast Centre
   • Pre-Admission Surgical Unit
   • Cardiology Clinics
   • Women’s and Infants’ Health Outpatient Clinics (3^rd floor – 700 University Ave)

2. Registration is also available:
   • If you are getting admitted as an inpatient or as a day surgery patient – ask at the Admitting Department
   • If you are a patient in the Emergency Department – ask at the Registration desk
   • If you are coming into give birth, ask at the Labour and Delivery Registration desk

3. Patients can also register at the Health Records Department Monday to Friday between 8:30 a.m. and 4:30 p.m. in two ways:
   • In-person (1^st floor, Room 182, 600 University Ave)
   • Over the phone (416-586-4800 ext. 2649) - We will ask you a series of questions to validate your identity.

• After you have signed-up for MyChart and given your email to registration, please look out for the PIN number in your inbox within 24-to-36 hours. If you cannot locate the email in your inbox, please check your spam/junk mail as well.
• Once you have received an e-mail with the 4-digit PIN number, launch your internet browser and in the address bar enter www.mychart.ca. Then, select REGISTER on the home screen.
• Search & Select Mount Sinai Hospital
• Input your First Name and Last Name as written on your Health Card, followed by Date of Birth, Gender, Health Card Number, and PIN number.
• Once you complete your registration, you can sign into MyChart anytime
• NOTES:
  • The PIN will be active for 10 days. If the PIN expires or to update your email, please contact MyChart Support at 1-833-221-2202 or [email protected].
  • We recommend that you use Google Chrome or Firefox browsers instead of Internet Explorer to view MyChart.

MyChart users can electronically grant access to family caregivers, hospital clinicians, primary care physicians, Home and Community Care, pharmacists, etc

For privacy reasons, it is best to register for MyChart under your own email address, so that you are in full control of your personal health information. If you do not have an email address and do not wish to sign up for an external email service, you can provide consent to register for MyChart under a family member or friend’s email.

Please keep in mind that when you provide access to a family member or friend, he/she will have ongoing access to your personal health information. This information includes sensitive details related to your healthcare from January 2014 and into the future

• Lab results including COVID-19 test result
• Diagnostic imaging reports
• Discharge summaries
• ECG/ECHO reports
• Operating room report
• Personal address book, compiled by the patients – physicians, caregivers, labs, clinics, etc.
• Personal diary and personal tools

MyChart is accessible from anywhere at any time through any device connected to the Internet. Once registration is complete users can go to www.mychart.ca and log in to review their MyChart records.

Test results and discharge summaries from Mount Sinai Hospital will be available in MyChart 3 days after the report has been produced.

MyChart is secured through the same encryption technologies used by the major banks for online banking.
