Telemedicine IMPACT Plus offers one-time inter-professional consultations to complex patients and their family physician to coordinate care planning and derive new solutions for addressing the patient’s chronic conditions. Across the Toronto Central LHIN, each TIP consulting team has a core membership of a psychiatrist, internist, pharmacist, social worker, and CCAC coordinator as well as a Psychogeriatric Resource Consultant for Primary Care as relevant.
Target population: Medically complex patients with multiple chronic conditions and medications, in need of access to psychiatric, mental health, or social supports
Who can make a referral: Primary care providers and physicians of patients with complex care needs within Toronto Central LHIN catchment area.
