What should I bring with me to my appointments?

Please bring your MSH card and Ontario Health Card with you to each appointment. If you don't have a MSH card, leave yourself extra time to get one from the Admitting Department (located on the main floor across from the Information Desk at the Murray Street entrance to the hospital).

Please bring your blood glucose meter and blood sugar records with you. Your blood sugar records are very important as they help the team and you understand how the different areas of your life are affecting your blood sugar control. We may recommend changes in medication or food using these records, so it is important that they be accurate and up-to-date.

You may be sent to the outpatient laboratory (located on the fourth floor) and asked to do a 'lab-meter' comparison which will check your meter's accuracy with the laboratory. You may have other blood tests done as well, depending on what your doctor feels is necessary to check other areas of your health. You will be notified if any of your test results are abnormal. If you wish to receive all of your test results, please bring a stamped self-addressed envelope with you to your appointment. We will be glad to send the test results to you.

If you have seen a dietitian in the past and received a meal plan, please bring it with you. It is also helpful to record what you eat for a three day period before your appointment. The dietitian will help you make a meal plan that is agreeable to your tastes and lifestyle.

You may have some questions about diabetes that you wish to ask the members of the health care team. Some of the questions commonly asked are:

  • What should I eat?
  • How do I manage sick days?
  • What do I do when I'm going to a big party?
  • How do I store my insulin? What if I'm travelling?
  • How do I adjust my insulin and meal plan when I travel?
  • What is Intensive Diabetes Management?

You may have many other questions. It is a good idea to write them down before your appointment so that you don't forget to ask them.

What if I need to change my appointment?

If you cannot attend your appointment, please call 416-586-4800 ext. 8747 as soon as possible.  This will allow us to reschedule your appointment and let another patient to use the time period you cannot.

Can anyone come to the appointment with me?

Absolutely! We encourage you to bring your family or friends with you. These people can be a source of support for you and may have their own questions about diabetes.
