Referrals or Requisitions
Healthcare Professionals can fax referrals to the following FAX lines:
Surgical Referrals: 416-586-8847
Breast Imaging / Mammograms: 416-586-4714
Ontario Breast Screening Program (OBSP) for Women at average risk for breast cancer for ages (50 to 74) for routine screening: 416-586-4714
OBSP for Women at High Risk ages (30 – 69) : 416-586-8847 (this referral goes directly to genetic counsellor who will assess eligibility for program).

Coming to the Centre
Then take the Murray Street elevators at the back, or west side, of the Hospital to the 12th floor.

Important: Before an appointment can be booked with one of the doctors, their office must receive a letter with appropriate relevant medical information (for example, test reports) from the doctor who is referring you.
The referring doctor may ask you to bring other test results, x-ray films etc. to your appointment.
Coming For Your Appointment
Please bring your Ontario Health Insurance Plan (OHIP) card and Mount Sinai Hospital card. If you do not have a Mount Sinai card please visit the Patient Registration Department on the main floor near the Murray St. entrance, before your appointment and get your card. If any of your personal information (i.e. name, address, phone number, version code on card etc.) has changed on your current card, please visit the Patient Registration Department and have a new card made. We suggest you allow yourself an extra 20 to 30 minutes before your appointment to get this done.
If you are seeing a doctor here for the first time please also bring a list of the current medications you are taking.
Breast Imaging Tests ( Mammograms etc.)

Important: You will be required to bring a requisition from your doctor when you come for your breast imaging test. A hospital card and valid health card must also be presented (please see Coming For Your Appointment).
