What to Bring

Being prepared for your visit helps ensure a smooth and comfortable experience. Click on the purpose for your visit from the list the below to see a list of suggested items to bring with you.

  • Your Ontario Health Insurance Plan (OHIP) Card
  • A list of all medications you are taking
  • A list of questions you may have

  • Your Ontario Health Insurance Plan (OHIP) Card
  • Identification showing your name, address and date of birth
  • Secondary insurance coverage information (if applicable)
  • All medication you are currently taking, in the original bottles, including vitamin or herbal supplements
  • Toiletry items such as a toothbrush, toothpaste, brush/comb, shaving equipment, shampoo and deodorant, cotton pyjamas with a button-down top, slippers, housecoat or dressing gown, glasses, hearing aids, and dentures (Electric razors and hair dryers are the only electrical grooming devices permitted)
  • You may wish to bring a small amount of cash, however, please leave valuables at home
