New Employee Orientation and Documentation
As part of your welcoming activities and orientation to the Hospital, you will complete the following:
- Pre-Employment Policy Review
- Payroll Documentation and Benefits Enrollment (as applicable)
- New Employee Orientation – 1 full day
- Health Review Questionnaire Mask Fit Testing
New Employee Orientation
The New Employee Orientation program is held at 60 Murray Street (just behind the main Hospital) in the 3rd floor conference room. This orientation day is mandatory and essential for your on-boarding at Mount Sinai Hospital. New employees are automatically registered as part of the new hire process by the Human Resources Department. Please refer to your offer of employment letter for the date that you have been scheduled.
The key elements of the introduction to the Hospital’s People, Resources and Workplace are:
Mandatory training and information including:
- Health and Safety, Infection Control, Workplace Hazardous Information Systems (WHMIS), Attendance Support Management, Fire Safety/Emergency Procedures, Hospital security, Interpreter Services, Human Rights, Health Equity, Patient Safety, and Risk Management
- Documentation and Photo ID badge
- Mask Fit Testing
- ONA Registration and SEIU Overview (as applicable)
- New Employee Information Form;
- Direct Deposit Form;
- TD1 – Federal Tax Form;
- TD1ON – Provincial Tax Form; and
- Employee Profile Information.
