Referrals

Please complete the Wasser Referral Form and Patient Intake Package. Sign the Referral Form and fax it back at the number provided, along with the Patient Intake Package, consultation notes and relevant imaging reports. Please refer to the Referral Form Instructions for help completing the form.

Please note we can only accept referrals from physicians and nurse practitioners.

After we have received and reviewed the referral, we will notify you of your patient’s appointment time. Please note, this will take approximately 4-6 months from the time we have received all of the completed documentation. We do not schedule appointments until we have the complete referral and the patient’s intake package.
     

Important Information

• All patients outside of the greater Toronto area will be seen for assessment and recommendations only.

• Patient must have a family physician willing to be an active participant in his/her care and provide
  the required follow-up.

• Patient must be 18 years of age or older. 

Requests for medicolegal purposes will not be accepted. Such referrals should be sent for an independent medical opinion with the explicit understanding that the referrals is for medicolegal purposes.
