H1N1 Planning - Bar Code Distribution – DEADLINE EXTENSION

October 19 - Thanks to everyone who has taken the time to obtain their bar code. There is still a large number of codes left to distribute. Consequently, we are extending the deadline to obtain a bar code until Saturday, October 24.

It is important to be prepared in the event that we need to dispense the H1N1 antiviral medication across the organization. It is mandatory that all active employees, physicians, fellows, residents and students (who have photo I.D. badges) have a bar code tag affixed to his or her Hospital I.D. badge. The bar code will serve as your personal identifier and provide us with the ability to record that the medication has been dispensed to you.

Commencing Saturday, October 10, you can obtain your bar code at any of the times listed below. Please be sure to bring your Mount Sinai photo I.D. badge with you, as bar codes cannot be issued to individuals without a valid Hospital I.D. badge.

Extended Dates:   

     Monday, October 19 to Saturday, October 24, 2009  
     (not available on the Thanksgiving Holiday – Monday, October 13)

     Monday, Wednesday and Friday 
     12:30 p.m. to 5 p.m. - 2nd floor cafeteria
     5:30 p.m. to 8:30 p.m. - 11th floor classroom

     Tuesday, Thursday and Saturday:
     7:30 a.m. to 12 noon - 2nd floor cafeteria
