Diabetes Education and Management
Do you have diabetes? Prediabetes?
You don't have to stop doing all the things you love to do to enjoy a healthier life - and we can help! The Mount Sinai Academic Family Health Team offers free diabetes education and management services including:
- Support and advice from a nurse experienced in diabetes care
- Nutrition counselling with a registered dietitian
- Opportunities to attend group sessions and learn from others who are living with diabetes
Our goals are to help you learn more about diabetes, set realistic goals and increase your ability to manage your health. Ask your primary care provider to refer you to our free program. External referrals welcome. Feel free to bring a family member, caregiver or friend.
