General Assessment Service

This service provides comprehensive outpatient assessment on a consultative basis. The assessment process may include a screening phone interview with the ambulatory intake coordinator, followed by an assessment which is generally done by a psychiatric resident and psychiatrist together.

Physician referrals are required. To make a referral, please call 416-586-4800 ext. 4568 or fax the referral form (83 KB pdf )  to 416-586-8654.

Physicians include Drs. Sian Rawkins, Lisa Andermann, Jared Peck, Steven Selchen, Rodney Slonim, Rebecca Esrock, MSW and Allison Hughes, MSW.

 
Participants

Participants must be 18 years and over with a variety of psychiatric/emotional problems.

Participants cannot take part if they have:

• an intellectual impairment
• current/pending criminal charges
• primary problems related to substance abuse
