The Psychogeriatric Resource Consultant for Primary Care program (PRC-PC)

The Psychogeriatric Resource Consultant for Primary Care (PRC-PC) program is part of the provincial Behavioural Support for Seniors Program to support the growing needs of our aging population. Our goal is to increase the capacity for care for older adults with psychogeriatric conditions, specifically those with dementia and other behavioural problems, at the primary care level, and throughout the health system.

We work directly with family doctors and primary care practitioners to make primary care a priority and create a more accessible health-care system for our aging patients and their families.

The PRC-PC works in collaboration with primary care practitioners and community services as well as other stake holders, to support and integrate primary care and home care.

Services

The PRC-PC serves primary care practitioners in the Toronto Central area through:

  • Providing case based consultation in complex cases;
  • Providing information about community resources;
  • Linking you to best practice, up-to-date information on related topics;
  • Researching and developing tools that can support and simplify your work;
  • Organizing learning opportunities as requested by each practitioner/team.
