Referral Criteria

The Menopause Clinic is a referral only clinic. Patients require referral by a medical practitioner. If you are the referring provider, please see patient referral criteria below.

Criteria

  • The patients are usually referred who have validated menopausal symptoms such as hot flashes, night sweats, mood changes, sleep difficulties or new sexual problems specifically related to menopause, or those who have abnormal uterine bleeding around the age of menopause.
  • Other reasons for referral may include:
    • Recommendations and best current practice regarding hormone therapy.
    • Managing menopause in complex medical situations.
  • The patient should have a negative pregnancy test.

Please include in the referral the following:

  • Copy of CPP
  • Relevant lab work and other investigations
  • Most recent pap test, if done
  • Most recent mammogram if done
  • Most recent bone density if done
  • Relevant clinical questions you would like us to address
