PRC - Dementia Resources for Primary Care

Welcome

Elderly women and caregiverThis is a website for primary care practitioners to support their work in providing care for older adults with dementia and responsive behaviours and their caregivers. In this website you will find tools, resources and information specifically designed for primary care settings. This program is part of the TC-LHIN Behavioural Support for Seniors Program, lead by the Reitman Centre for Alzheimer's Support and Training and by the PRC Program of Toronto, in partnership with the Regional Geriatric Program.
